Anyone
taking treatments is required to accept the following terms
and conditions.
1.
I
reserve the right to refuse treatment. I will not treat anyone
who has or is recovering from an infectious illness or fever.
Neither will I treat anyone who is pregnant or suspects that
she may be pregnant.
2.
You
will need a doctor's note stating his or her permission for
you to have treatment if you suffer from any of the following
conditions :
>
Surgery within the last six months
>
Thrombosis or embolism
>
Cancer, tumour or HIV/AIDS
>
Very high or very low blood pressure
>
Heart or circulation problems
>
Diabetes
>
Epilepsy
3.
Further
to those conditions stated in paragaraph 2, Hopi candle treatments
cannot take place if there is any evidence of the following
conditions:
>
Ear infection
>
Grommets
>
Glue ear
>
Perforated ear drum
>
Head or neck injury
4.
All
clients are required to answer fully and to the best of their
knowledge the questions regarding medical background and lifestyle.
This is so that I am able to make informed decisions regarding
treatment. Any clients withholding medical or lifestyle information
does so at their own risk. Furthermore, I will not be responsible
for any untoward events happening during or after a treatment
which could have been avoided had clients not withheld information.
5.
All
clients are required to sign a form stating that they have supplied
the relevant medical and lifestyle information, they have understood
any contra-actions associated with the treatment and are happy
to go ahead with the treatment.
6.
All
children being treated must be accompanied by a parent or guardian
at all times.
7.
While
I am present at your location, you must have sight of me at
all times.
8.
All
treatments are to be paid for in full at the beginning of the
treatment session. Treatments are payable by cash or cheque
only. No other payment method will be accepted.
9.
Cancellations.
If you miss your appointment on the day of the treatment then
the full charge is payable. If you give 24 hours notice of cancellation
then only half the charge is payable. If you give more than
24 hours notice of cancellation then no charge is payable. These
charges are necessary because it is highly unlikely that I will
be able to cover your slot with another client in such a short
period.
10.
For
Hydrotherm and armomatherapy massage, a chaperone (e.g. your
partner, friend, etc.) must be present in the room or within
earshot of the room in which the massage is taking place.
11.
Clients
making any suggestion of an improper nature or showing over-familarity
will result in me stopping treatment and leaving the location
immediately without refund. Any existing bookings will be cancelled
and no further bookings will be taken.
